Request for UB Identification (ID) Photo Non-Release

The university uses UB ID photos in university information systems for approved educational and business purposes, including security and identification. Prior to a student ID photo being used for social purposes (i.e., other than for security or identification purposes) the student must have given consent. Unless otherwise notified in writing, the University at Buffalo has your consent to use ID photos for all purposes determined to be appropriate by the University; related to enrollment, identification, or security. A student may decline the use of their ID photo for social purposes only by completing this form.

Please note that all information and your signature must be included for opting out of ID photos being used for social purposes.
